Projects (historical research or rehabilitation) or leadership activities should have been completed in the period from late May\/June 2011 to May 2012 to be eligible for this year&#8217;s round of awards. <\/p>\n<p>Download a nomination form at <a href=\"http:\/\/www.phwi.org\/issues\/award.pdf\">www.phwi.org\/issues\/award.pdf<\/a>.
